HRT for menopause in the Philippines: what is sold, what it costs, how to get it
Editorially reviewed on October 29, 2026Medical review: pending
Quick answer
Menopausal hormone therapy (HRT) is estrogen, plus a progestogen if you still have a uterus. In the Philippines it is prescription only, and the basics are on Watsons shelves: estradiol gel, micronized progesterone, dydrogesterone, tibolone, conjugated estrogens and vaginal estrogen. For most healthy women with bothersome symptoms who are under 60 or within 10 years of menopause, The Menopause Society (2022) says the benefits outweigh the risks. You need a consult and a prescription first; patches and combined tablets were not found in PH retail.

First, a note on the word "HRT"
If you typed "HRT" into Google in the Philippines, you may have landed on threads about gender-affirming hormone therapy. A lot of local "hrt" searches are exactly that, and that care matters. This page is about something else: hormone therapy for perimenopause and menopause. Doctors also call it MHT, menopausal hormone therapy. Same idea, different name.
If you are looking for gender-affirming care, a doctor experienced in it is the right first stop, not this guide.
What HRT for menopause actually is
Around menopause your ovaries make much less estrogen. That drop is behind hot flashes, night sweats, the 3 am waking, vaginal dryness and faster bone loss. HRT puts back a steady, low dose of estrogen.
There are two parts:
- Estrogen does the work on symptoms. In the Philippines that usually means estradiol gel rubbed on the skin, or conjugated estrogens as a tablet.
- A progestogen protects the womb lining. Estrogen alone makes the lining thicken, which raises the risk of womb (endometrial) cancer. If you still have a uterus, you need both. If you have had a hysterectomy, estrogen alone is usually enough.
Tibolone is the exception: one tablet that does both jobs. Vaginal estrogen is another category again: a low local dose for dryness and bladder symptoms, which most women can use without a progestogen.
The Menopause Society 2022 position statement calls hormone therapy "the most effective treatment for vasomotor symptoms" (that is, hot flashes and night sweats), and says it also prevents bone loss and fractures.

Notes worth knowing:
- The Duphaston listing showed "Notify Me", so it may be out of stock at times.
- The tibolone pack size is not stated on the listing, so we cannot give a per-tablet figure yet.
- No 100 mg micronized progesterone listing was found. The UK Utrogestan label lists 100 mg at bedtime on days 1 to 25 of each cycle as an alternative to 200 mg for 12 days, so ask your doctor how a 200 mg capsule fits your regimen.
- Want the monthly total for an example regimen? Use the HRT cost calculator instead of doing the math by hand.
Each medicine has its own guide: estradiol gel, micronized progesterone, dydrogesterone, tibolone, conjugated estrogens, medroxyprogesterone, vaginal estradiol and estriol cream.
What is not sold in PH retail (yet)
Some common HRT products abroad did not turn up at any Philippine retailer we searched:
- Estradiol patches (Estradot, Climara, Evorel): Not available in PH retail as of October 2026
- Estradiol plus dydrogesterone tablets (Femoston): Not available in PH retail as of October 2026
- Oral estradiol at menopause doses (Progynova): Not available in PH retail as of October 2026
- Tibolone, Livial brand: Not available in PH retail as of October 2026 (Amena is the brand we found)
- Conjugated estrogens, Premarin brand: Not available in PH retail as of October 2026 (a tablet with no stated brand is sold)
- Fezolinetant (Veozah), the newer non-hormonal pill: Not available in PH retail as of October 2026
"Not found" is not the same as "not registered". We have not yet checked each one against the FDA Philippines registration database, so ask your doctor or pharmacist. If you were on a patch abroad, the expat guide covers switching to gel with your doctor and the personal-import rules for bringing your own supply.
Who HRT is for
The Menopause Society 2022 position statement says that for most healthy women with bothersome symptoms who are under 60 or within 10 years of menopause, the benefits of hormone therapy are greater than the risks. It also says there is no need to stop routinely at 60 or 65; the decision is individual.
That usually means HRT is worth discussing if:
- Hot flashes, night sweats, poor sleep or mood changes are affecting your work, family or relationships.
- You had POI or early menopause (before 45). The Menopause Society says hormone therapy should be used until at least the average age of natural menopause, unless there is a medical reason not to.
- You have vaginal dryness, pain with sex or bladder symptoms (vaginal estrogen may be all you need).
- You are at risk of bone loss and other options do not suit you.
At the October 2025 UP Manila symposium, PGH OB-GYN faculty put it simply: "You start HRT as soon as the symptoms show up on your patient or within five years of diagnosis" (UP Manila).
HRT is usually not suitable if you have, or have had, breast cancer or another estrogen-dependent cancer, a blood clot in the legs or lungs, a stroke or heart attack, active liver disease, or bleeding that has not been checked. High blood pressure is not an automatic no if it is controlled, and skin gel may be a better fit than tablets for some women, since the transdermal route may carry less clot and stroke risk (The Menopause Society 2022). This is a doctor's call, made with your history.

Sequential vs continuous: the two ways to take it
If you have a uterus, the progestogen can be given two ways.
Sequential (cyclic): estrogen every day, plus a progestogen for the last 10 to 14 days of each cycle (NHS). You usually get a light, predictable bleed each month. This suits women who are still having periods, even irregular ones, or whose last period was less than about a year ago.
Continuous combined: estrogen and a progestogen every day, with no break. The aim is no bleeding at all, although irregular bleeding is common for the first few months (NHS). Bleeding that is heavy or unexpected after 6 months needs checking. This usually suits women at least a year past their last period.
Tibolone works like a continuous regimen in one tablet and is only for women whose periods stopped at least a year ago.
Your doctor picks the regimen and dose. The examples in our cost calculator are examples only.
How to get an HRT prescription in the Philippines
Every product above is prescription only. Here is the usual path:
- Book a consult. An OB-GYN is the most common route, and some internists and family doctors also prescribe. Teleconsults are an option: specialist teleconsults are ₱699 per 15 min (Doctor Anywhere, Medifi). In-person OB-GYN fees range from ₱300 to ₱600 per visit (Government hospitals) to ₱600 to ₱2,000 per visit (St. Luke's, Makati Med, TMC, Cardinal Santos, Healthway). Prices checked October 2026.
- Bring your story, not just your labs. Your age, when your periods changed, your top three symptoms and how they affect your days. A symptom diary helps. Over 45 with typical symptoms, you usually do not need hormone blood tests to diagnose perimenopause or menopause (NICE NG23 and QS143).
- Expect a few checks. Blood pressure, weight, a breast exam or mammogram if due, and questions about clots, cancer and bleeding.
- Get the prescription, paper or electronic. E-prescriptions were allowed during COVID and later extended (Philstar). Rules may have changed since, so ask your doctor and pharmacy what format they accept.
- Fill it. In store at any pharmacy, or online at Watsons: add the item to your cart, check out, then email a copy of your prescription with your order number to OnlinePharmacy@watsons.com.ph. The item ships after the pharmacy checks it (as stated on Watsons Rx product pages).
- Follow up in about 3 months. The NHS says a review 3 months after starting (or changing) HRT is usual, then once a year if all is well.
We never sell prescription medicines and we do not link to any "buy" page for them. Our find a doctor page lists clinics and community-recommended doctors.
"Ayaw akong resetahan ng OB ko"
This is the most common story we read. Filipinas on Reddit report OBs who "do not prescribe", dismiss symptoms as stress, or simply change the subject (r/AskPinay). One poster wrote that a lot of women who ask for HRT "are denied".
Some doctors still practise by the scary headlines of 2002. Guidance has moved on since then. If you hear "no", here is what you can do:
- Ask why. "Is there something in my history that makes HRT unsafe for me?" is a fair question. If there is a real reason, such as a past clot, you want to know it.
- Bring the guideline. The Menopause Society 2022 says benefits outweigh risks for most healthy women under 60 or within 10 years of menopause. Bring that line, printed.
- Bring your diary. Two to four weeks of hot flashes, sleep and mood notes are hard to wave away. Our talk to your doctor page has a Taglish script.
- Ask about other options. If HRT really is not for you, ask about non-hormonal treatments such as paroxetine, gabapentin or CBT.
- Get a second opinion. That is normal, not rude. The Philippine Society of Climacteric Medicine (PSCM) has a "Find a Menopause Clinic" page (PSCM LAYAG), and our find a doctor page lists menopause clinics.
Being matiisin is a virtue in many things. Sleeping three hours a night for five years is not one of them.
What about "bioidentical" hormones?
Good news: two of the main registered products are already bioidentical. Estradiol gel is the same estradiol your ovaries made, and micronized progesterone is the same progesterone. Both are FDA-registered, dosed and labelled.
Custom "compounded" bioidentical creams and pellets are different. The Menopause Society (2022) and ACOG (2023) both advise against them when an approved product exists. The full story is on our bioidentical hormones page.
What to expect once you start
- Hot flashes and night sweats should improve after a few days or weeks (NHS). Your 3-month review is the time to judge the dose.
- Sleep, mood and joint aches may improve too, partly because nights get calmer.
- Common early side effects are breast tenderness, bloating, headaches and spotting. They often settle in the first months. Tell your doctor if they do not.
- Vaginal symptoms may need vaginal estrogen on top; NICE (2024) says it can be used alongside systemic HRT for as long as needed. See vaginal estrogen.
See a doctor promptly for bleeding after menopause that is new or heavy, a breast lump, chest pain, a painful swollen leg, or a sudden severe headache.
Frequently asked questions
How much is HRT in the Philippines?
It depends on the regimen. At Watsons, estradiol gel is ₱1,185.50 per 80 g pump pack (Watsons) and micronized progesterone is ₱85.75 per capsule (Watsons). Prices checked October 2026. Our cost calculator adds up example monthly regimens, plus consult and lab costs.
Can I buy HRT without a prescription?
No. Every HRT product in the Philippines is prescription only, including vaginal estrogen. Watsons online asks you to email your prescription to OnlinePharmacy@watsons.com.ph before it ships. Avoid marketplace sellers: you cannot check what you are getting.
What is the best HRT for menopause?
There is no single best one. Many doctors start with estradiol gel plus micronized progesterone, partly because the skin route may carry less clot risk (The Menopause Society 2022). Tibolone suits some women after periods stop. Your history decides.
Is it too late to start HRT at 55 or 60?
Not necessarily. The Menopause Society 2022 says benefits are most favourable under 60 or within 10 years of menopause. Starting later is possible but needs a more careful talk about heart and clot risk with your doctor.
Do I need blood tests before HRT?
Usually not to diagnose menopause if you are over 45 with typical symptoms (NICE NG23 and QS143). Your doctor may still check blood pressure, a mammogram and other routine tests. See our FSH test guide.
Does HRT make you gain weight?
Midlife weight gain, especially around the belly, is common with or without HRT. Bloating can happen in the first weeks. See weight and belly changes for what helps.
Pwede ba ang HRT kung may highblood ako?
Controlled high blood pressure is not an automatic no. Skin gel may be preferred over tablets because the transdermal route may carry less clot and stroke risk (The Menopause Society 2022). Your doctor decides after checking your numbers.
