CONSULT PREP

How to talk to your OB about menopause (and actually be heard)

Editorially reviewed on October 29, 2026Medical review: pending

Quick answer

Bring a short symptom diary, a list of what you have tried, and one clear request, such as "I'd like to discuss HRT" or "I'd like to know my options". Doctors respond best to specific symptoms with dates and how they affect your work and sleep. If you are told it is "just stress", it is fair to ask what else it could be and whether perimenopause has been considered.

When your OB says it's just stress

I understand stress plays a part. But these symptoms started with changes in my periods, and they are affecting my work and sleep. Could this be perimenopause? I'd like to talk about treatment options.

When you want to discuss HRT

I've read the current guidance from The Menopause Society. I'm under 60 and my symptoms are bothering me. Could we discuss whether HRT, like estradiol gel with progesterone, is right for me? If not, why not, and what else could I try?

When you want a second opinion

Thank you. I'd still like to explore this further. Could you refer me to a doctor who focuses on menopause?

Educational only. Not a diagnosis or prescription.

Why this visit feels hard

Many Filipinas walk into a consult already apologising. We were raised to be matiisin, and a woman with many complaints gets labelled "maarte" or "OA", as one woman put it on r/AskPinay. Others report hearing that their symptoms are "muscle pain, migraine or stress", and leaving with only a pain reliever.

A 15-minute consult moves fast. The women who get the most out of it come prepared: they bring dates, not just feelings, and they say what they want to talk about. One Redditor who kept a journal for her doctor ended up with the treatment she needed. Preparation is not being pushy. It is helping your doctor help you.

Before the visit: your 10-minute prep

  1. Keep a diary for 2 to 4 weeks. Log hot flashes, night sweats, sleep, mood, period changes and anything odd (itchy ears, joint pain, palpitations). Our free symptom diary does this on your phone and makes a one-page summary. Prefer paper? Print the symptom diary.
  2. Write your top three symptoms and how each affects your life: "I wake at 3 am four nights a week and I'm making mistakes at work."
  3. Note your periods: date of the last one, how cycles have changed, any heavy or unusual bleeding. The period tracker helps.
  4. List medicines and supplements you take, including vitamins and herbal products.
  5. Know your history: any breast cancer, blood clots, stroke, heart disease, liver disease or migraine with aura, in you or close family.
  6. Decide your one request. For example: "I'd like to discuss HRT", "I'd like to know non-hormonal options", or "I want to know if this is perimenopause or thyroid."

You can also run the symptom checker and bring the printout. It shows how much each area bothers you; it is not a diagnosis.

Script: "My OB says it's just stress"

Script: "I'd like to discuss HRT"

During the visit

  • Lead with your top three symptoms and their impact. Hand over your diary summary.
  • Say your request early, not as you stand up to leave.
  • Ask for numbers. "What is my actual risk?" is a fair question. Our page on HRT and cancer explains the evidence in plain language.
  • Ask what the plan is: what to try, how long before it works, when to follow up.
  • Write things down or bring someone. Daughters and partners make great note-takers; see paano tulungan si Mama.

Useful questions:

  • "Is this perimenopause, or should we check thyroid too?"
  • "Do I need any tests, given my age?"
  • "What are my hormone and non-hormonal options?"
  • "If I still have my uterus, which progestogen would I need?"
  • "How much will this cost per month?" (Our cost calculator shows example regimens.)

If it still does not go well

You can ask for a second opinion. That is normal practice, not an insult. Some women find a better fit with a doctor who has a special interest in menopause; see find a doctor for the Philippine Society of Climacteric Medicine, hospital menopause services and teleconsult options.

Whatever happens, some symptoms should never wait for the next appointment: bleeding after menopause, a breast lump, chest pain, sudden severe headache, leg swelling, or thoughts of harming yourself. See a doctor soon.

Frequently asked questions

Paano ko sasabihin sa OB ko na gusto ko ng HRT?

Start with your symptoms and how they affect you, then say it plainly: "Gusto ko po sanang pag-usapan ang HRT." Bring a symptom diary. Use the scripts on this page.

My OB says it's just stress. What do I say?

Agree that stress is possible, then ask whether perimenopause has been considered given your age and cycle changes, and what else could explain your symptoms. Show dated notes from your diary.

Do I need blood tests before asking about HRT?

Often not. NICE NG23 says perimenopause and menopause can usually be diagnosed without blood tests in women over 45 with typical symptoms (NICE NG23 and QS143). Your doctor may still order tests for other reasons.

What should I bring to a menopause consult?

A 2 to 4 week symptom diary, your period dates, a list of medicines and supplements, your personal and family medical history, and one clear request.

Is it rude to ask for a second opinion?

No. Second opinions are a normal part of medical care. If your concerns are dismissed without explanation, it is reasonable to see another doctor.
