MEDICINE GUIDE

Gabapentin for hot flashes and night sweats

Editorially reviewed on October 29, 2026Medical review: pending

Quick answer

Gabapentin is a nerve-pain and seizure medicine that can also reduce hot flashes. The Menopause Society 2023 nonhormone statement lists it as a recommended option. Because it makes many people sleepy, doctors often prescribe it at bedtime for night sweats. It is prescription only; the 300 mg capsule costs ₱42 per capsule (Watsons).

Key facts

  • What it is: gabapentin, originally a medicine for seizures and nerve pain.
  • Rx status: prescription only.
  • Sold in PH: gabapentin 300 mg capsule (Reinin), ₱42 per capsule (Watsons). Prices checked October 2026.
  • Used for in menopause: vasomotor symptoms, especially night sweats that wake you up. In the US, gabapentin is approved for seizures and nerve pain, not hot flashes, so this is an off-label use; only paroxetine 7.5 mg and fezolinetant are US-approved non-hormonal hot flash medicines (Menopause 2023). We could not check the Philippine leaflet.
  • Not a hormone: it does not protect bones or help vaginal dryness.

How gabapentin is used for hot flashes

The Menopause Society 2023 nonhormone position statement recommends gabapentin alongside SSRIs and SNRIs, fezolinetant, oxybutynin, CBT, clinical hypnosis and weight loss (Contemporary OB/GYN). The same statement does not recommend pregabalin, a related medicine, for hot flashes.

Doctors usually start low and increase slowly, often with the first dose at bedtime. The full 2023 statement notes that because drowsiness is a side effect and the medicine wears off quickly, bedtime dosing may suit women whose hot flashes break their sleep (Menopause 2023). Your doctor sets the dose and how fast to increase it.

Gabapentin is worth discussing if you:

  • Cannot take hormones (for example after breast cancer or a blood clot)
  • Are on tamoxifen and cannot take paroxetine
  • Mostly struggle with night sweats and broken sleep

Who should not use it, or should check first

Tell your doctor before starting if you (MedlinePlus; US FDA 2019):

  • Have kidney disease or lung disease (such as COPD)
  • Take opioid painkillers, sleeping pills or other medicines that cause drowsiness
  • Drink alcohol (it adds to the drowsiness)
  • Have a history of depression or other mental health concerns, or of misusing medicines
  • Are pregnant, planning a pregnancy or breastfeeding

Side effects

The most common side effects are drowsiness, dizziness and feeling unsteady; in hot flash trials these were usually worst in the first week and settled by about week 4 (Menopause 2023). MedlinePlus also lists swelling of the hands, feet or legs, weight gain, and blurred or double vision.

  • Do not drive or operate machinery until you know how it affects you.
  • Older women should be careful about falls, especially at night.
  • Do not stop suddenly. Stopping at once can cause anxiety, poor sleep, nausea, pain and sweating; doctors usually lower the dose over at least a week (MedlinePlus).

See a doctor soon for thoughts of self-harm, a rash with fever, or trouble breathing. Leg swelling on one side with pain can be a blood clot and needs urgent care.

Strengths of gabapentin: no hormones, no listed interaction with tamoxifen (Drugs.com interaction checker), and the drowsiness can be useful at night.

Trade-offs: daytime sleepiness and dizziness, and it does nothing for vaginal dryness, mood or bones. Hormone therapy is still the most effective treatment for hot flashes (The Menopause Society 2022), so if hormones are safe for you, it is worth asking about them too.

Questions to ask your doctor

Bring your hot flash counter or symptom diary so your doctor sees the pattern. Then ask:

  • Is hormone therapy an option for me, or is there a reason to avoid it?
  • Should I take gabapentin only at night, or also in the day?
  • How slowly will we increase the dose, and when do we review it?
  • Do any of my current medicines add to the drowsiness?
  • Is my kidney function fine for this medicine?
  • How do I stop it safely if it does not help?

Alternatives

  • Paroxetine: an SSRI that can also help mood.
  • Fezolinetant: a newer non-hormonal drug, not found in the PH yet.
  • CBT for hot flashes and sleep (NICE 2024, Menopause Society 2023).
  • Hormone therapy, if it suits you: see HRT for menopause.

Compare all options on non-hormonal treatments, then find a doctor.

Frequently asked questions

Does gabapentin work for hot flashes?

The Menopause Society 2023 lists it as a recommended non-hormonal option. It works less well than hormone therapy but is a real option when hormones are not suitable.

Why take gabapentin at night?

It often causes drowsiness. The 2023 Menopause Society statement suggests bedtime dosing may suit women whose hot flashes disrupt sleep.

Can I take gabapentin with tamoxifen?

Paroxetine and fluoxetine can block the enzyme (CYP2D6) that turns tamoxifen into its active form (Menopause Society 2023). Gabapentin does not work through that enzyme, and a standard interaction checker lists no interaction with tamoxifen (Drugs.com). Confirm with your oncologist.

Is gabapentin addictive?

It can be misused. In 2019 the US FDA warned that gabapentin with opioids or other sedating medicines can cause serious breathing problems, especially in older adults and people with lung disease, and asked makers to study its abuse potential. Take it only as prescribed and tell your doctor about other medicines.

Pregabalin vs gabapentin for hot flashes?

The Menopause Society 2023 recommends gabapentin but not pregabalin for hot flashes.
