Bleeding after menopause: why even a little spotting needs a doctor

Editorially reviewed on October 29, 2026Medical review: pending

Quick answer

Any vaginal bleeding that happens 12 or more months after your last period should be checked by a doctor, even if it is light, brown, pink or happened only once. Most of the time the cause is not serious, but bleeding after menopause can be an early sign of cancer of the womb lining, so it should not be ignored. Expect a pelvic exam and usually a transvaginal ultrasound, and sometimes a small sample of the lining (biopsy).

Bleeding after menopause check

Two questions.

Has it been 12 months or more since your last period?
Have you had any bleeding or spotting since?

Educational only. Not a diagnosis or prescription.

Nilalabasan pa ba kahit menopause na?

Once you have gone 12 months in a row without a period, you have reached menopause (UP Manila). After that, your womb should not bleed on its own.

So if you see blood after that point, it is not "a late period", not "naligaw na regla", and not something to wait out. It needs a check-up, even if:

  • it was just spotting on your underwear,
  • it was brown or pink rather than red,
  • it happened once and stopped,
  • you feel completely fine otherwise.

This is one of the few rules in menopause care with no exceptions. PGH OB-GYN faculty stressed exams to rule out cancer in women over 45 at the 2025 UP Manila symposium (UP Manila).

What can cause it

Most bleeding after menopause turns out to have a benign cause. Common ones include (NHS):

  • Thinning and dryness of the vaginal or womb lining from low estrogen (GSM).
  • Polyps: small growths in the womb or cervix, usually not cancer.
  • Thickening of the womb lining (endometrial hyperplasia), which can sometimes become cancer if left untreated.
  • Hormone therapy: some bleeding can happen in the first months of HRT, especially with certain regimens. It still needs to be reported to your doctor, who decides whether it needs checking.
  • Cancer, such as cancer of the womb lining (endometrium) or ovaries. This is less common, but it is the reason every episode gets checked. Caught early, it is very treatable.

You cannot tell which cause it is from how the bleeding looks. That is why the tests matter.

Filipinas on Reddit have shared stories of mothers who bled after menopause and waited, including one later diagnosed with advanced cervical cancer (r/adultingph). These are personal accounts, not statistics, but they show why early checking matters.

What to expect at the doctor

Your OB-GYN will usually:

  1. Ask questions: when it happened, how much, how often, whether you are on hormones or blood thinners, and your family history.
  2. Do a pelvic exam, which may include a speculum exam and a Pap or HPV test if you are due.
  3. Order a transvaginal ultrasound (TVS). A small probe is placed in the vagina to measure the thickness of the womb lining. It is quick and usually not painful.
  4. Possibly take a biopsy. If the lining looks thick or the cause is unclear, the doctor may take a small sample of the lining. This can be done in the clinic, or with a hysteroscopy (a thin camera into the womb) in a procedure room.

The ultrasound result guides the next step. A thin lining is reassuring; a thicker lining or ongoing bleeding usually leads to a biopsy, often taken during a hysteroscopy (NHS).

How much it costs

  • OB-GYN consult, government hospital: ₱300 to ₱600 per visit (Government hospitals)
  • OB-GYN consult, private hospital: ₱600 to ₱2,000 per visit (St. Luke's, Makati Med, TMC, Cardinal Santos, Healthway)
  • Transvaginal ultrasound: ₱1,500 per scan (Exact Check)
  • Transvaginal ultrasound at other labs: ₱1,800 to ₱3,200 per scan (Hi-Precision, Healthway (estimates))

Biopsy and hysteroscopy costs vary a lot by hospital and whether anaesthesia is needed, so we do not list them. Ask the clinic for a quote. If cost is a worry, government hospitals such as PGH have OB-GYN clinics at lower fees.

Do not let cost delay the first visit. A consult and an ultrasound are the main first steps.

Prices checked October 2026. See costs for more.

What about bleeding during perimenopause?

Before menopause is confirmed, irregular periods are expected: cycles get shorter or longer, and you may skip months. But some patterns still need a doctor:

  • Very heavy bleeding (soaking through pads quickly, large clots, flooding).
  • Bleeding between periods that keeps happening.
  • Bleeding after sex.
  • Periods lasting much longer than usual.
  • Feeling dizzy, short of breath or very tired, which can mean anaemia.

Track your cycles with the period tracker, which flags these patterns. Read more on irregular periods.

If you are helping your mother

Many mothers will not mention bleeding because they are embarrassed or do not want to be a burden. If Mama mentions spotting, or you see signs of it, gently encourage a check-up and offer to go with her. You might say: "Ma, sabi ng doktor, kahit konting dugo after menopause, dapat ipa-check. Samahan kita." Our guide for families has more: paano tulungan si Mama.

Find a doctor on our find a doctor page, or book a first consult through NowServing or Hati Health. For bleeding, an in-person visit is best, because you will need an exam and an ultrasound.

Frequently asked questions

Is bleeding after menopause normal?

No. Any bleeding 12 or more months after your last period should be checked by a doctor, even if it is light or happened only once. Most causes are not serious, but cancer has to be ruled out.

I only spotted once. Do I really need to see a doctor?

Yes. Even a single episode of light spotting after menopause should be checked. How much you bleed does not tell you the cause.

Nilalabasan pa ba ang babae kahit menopause na?

After 12 months without a period, the womb should not bleed on its own. Any bleeding after that point needs a check-up.

I am on HRT and had some bleeding. Is that okay?

Some bleeding can happen in the first months of hormone therapy, depending on the regimen. Always tell your doctor. They will decide whether it needs an ultrasound or other tests.

What test is done for postmenopausal bleeding?

Usually a pelvic exam and a transvaginal ultrasound, which costs about ₱1,500 at Exact Check. If needed, the doctor takes a small sample of the womb lining (biopsy). Prices checked October 2026.

Can stress or heat cause bleeding after menopause?

Do not assume so. Bleeding after menopause has a physical cause that needs to be found. See a doctor rather than waiting for it to happen again.
